Notifications: Disable notifications with mandatory comment

We need the information tracking of disable notifications in the GUI, so that the user has to add an comment with the date, why notifications are disabled.
This is required by law in the banking and insurance section, all changes has to be tracked.
Please implement it like the acknowledge information icon (date,user, comment) and hover info that all changes done by user or admins are verifiable in the GUI and log.
